Dozens of bottles of Moderna COVID-19 vaccine had to be thrown away because of “human error,” Aurora Health Care officials said.
Lawyer Aurora Health officials said the incident happened over the weekend, when an employee took the bottles out of the Grafton Clinic refrigerator to access something else and did not return them.
“We found out that about 50 ampoules of Moderna vaccine were inadvertently removed from a pharmacy refrigerator overnight,” a spokesman said. “While part of the vaccine was administered to team members on December 26 in the approved window 12 hours after refrigeration, unfortunately most of it had to be discarded due to the temperature storage requirements needed to maintain its viability. . “
The health care system said 17,000 team members had been vaccinated in the past 12 days.
“We are clearly disappointed and regret that this has happened,” a spokesman said.
