A 91-year-old amputee from Ohio went into shock, collapsed and almost died after accidentally receiving the second dose of COVID-19 Modern vaccine twice just four hours apart.
Victor Smith of the Cincinnati suburb of Hamilton received the first dose of the Modern vaccine at Fort Hamilton Hospital in January, according to the Cincinnati Enquirer.
He went to get his second dose of the vaccine at Butler County Fairgrounds on Feb. 25, his daughter, Dawn Smith-Theodore, wrote on Facebook.
At the fair, Smith received a vaccination card with his first name written on it, but an incorrect last name, according to his daughter.
“As it turns out, he was a Victor, but he was a different family name,” Smith-Theodore said.
After receiving the second dose, he was sent to the Jamestowne Rehab unit in Hamilton, run by Community First Solutions.

Four hours after receiving the second blow, a fire officer came to the unit to administer the vaccine to a man of the same name.
“At 3:45, a firefighter came in and said they were shot for Victor and said ‘Victor Smith’ and he said ‘yes,'” Smith-Theodore told WKRC-TV.
And they went into my father’s room. Nobody went with them.
“They were there for nine minutes and gave another vaccine to Moderna.”
She added: “My father said ‘I have two vaccines.’
Moments later, Smith-Theodore received a phone call informing her that her father was suffering from respiratory distress and that he had been rushed to Fort Hamilton Hospital.
She said she was led to believe her father’s life was in danger.

After receiving the second dose, he was sent to the Jamestowne Rehab unit in Hamilton, run by Community First Solutions.
“Yesterday, when I spoke to my father and he was able to mutter two words – it was heartbreaking to me because I didn’t have my vaccine, so I can’t come to see him,” said Smith-Theodore, who lives in Calabasas. California, said March 1.
“So I haven’t seen my father in a year.”
Fortunately, his condition has improved and he is now recovering at the hospital.
“I want to see my father improve. I want to see that this doesn’t happen to anyone else, “said Smith-Theodore.
City of Hamilton and Community First Solutions issued a statement saying: “There was an incident in which a Hamilton resident accidentally received two doses of COVID-19 vaccine on the same day.
The person was a Jamestowne rehabilitation patient where all vaccinations are completed by outpatients, who are properly trained and certified, in this case, the Hamilton City Fire Department.

Four hours after receiving the second blow, a fire officer entered the unit to administer the vaccine to a man of the same name.
Jamestowne staff recognized the patient’s signs of suffering and responded immediately.
“The patient was transported to the hospital and the family was contacted promptly.
“Both Jamestowne and the Hamilton Fire Department have begun investigations into the incident.
“Our thoughts are with the patient and his family as he continues to recover.”
